[bs_col class=”col-sm-8″]Have you completed or scheduled your annual wellness exam with your primary care provider?  Now is the time to schedule in order to avoid a delay in receiving your discount (or not being eligible to receive the WellU discount).  Requirements must be completed by June 30 in order to qualify.  Discounts will begin when completions are reported by the health plan or community partners (they are not retroactive).

Employees must complete the following:

  • Annual physical or Wellness Exam between July 1, 2015 and June 30, 2016
  • General Health Assessment between July 1, 2013 and June 30, 2016 (if you are currently participating in WellU, you don’t need to complete the GHA again this year)
  • Two WellU wellness activities between July 1, 2015 and June 30, 2016

See the online brochure for WellU Wellness Activity options and additional information.[/bs_col]
