On-the-Job Injury Reporting

Supervisors play a critical role in reporting on the job injuries. They should report injuries as soon as possible after the injury. Human Resources Absence Management coordinates worker compensation issues.  In certain circumstances, Occupational and Environmental Health and Safety is required to notify Utah Occupational Safety and Health of workplace injuries. Circumstances requiring reporting include:

  • Work-related fatalities, within eight hours of finding out about them.
  • Work-related inpatient hospitalizations of one or more employees, within 24 hours.
  • Work-related amputations, within 24 hours.
  • Work-related losses of an eye, 24 hours.
  • Other significant injuries

Should a university employee experience an on-the-job injury or illness, especially those requiring reporting as listed above, OEHS must be notified, as soon as possible, by calling 801-581-6590 or (after business hours) 801-585-2677 and request the OEHS on-call.  In some instances, it will be necessary to secure the incident scene for an independent investigation.

When contacting OEHS, be prepared to provide the following information if possible:

  • Location of the work-related incident
  • Time of the work-related incident
  • Type of event (i.e., fatality, inpatient hospitalization, amputation or loss of an eye, etc.)
  • Number of employees injured
  • Names of the employees injured
  • Contact person (supervisor) and their phone number
  • Brief description of the incident including names of witnesses
