Health risk assessment for WellU participants

University Human Resource Management and the Osher Center for Integrative Health are excited to announce the launch of the WellU Health Risk Assessment (HRA) on September 3rd! This assessment is the first step to participating in the WellU Wellness Program for academic campus and U Health academic employees.  (This is not applicable for Hospitals and Clinics employees.) The HRA is designed to provide valuable insights into your health and wellness and provide a gateway to guidance and assistance if you need and want it.

To participate in WellU next Plan Year (July 1, 2025 through June 30, 2026), you must complete the new WellU HRA (the Regence assessment will not count for next year).

How to access the HRA

All benefit-eligible employees will receive an email on Tuesday, September 3, from wellness@utah.edu with a display name of Center for Integrative Health.  The email will contain a unique login to access the WellU HRA. Please watch for this email in your inbox and follow the instructions to complete your assessment. Be sure to check your junk mail folder if you have not received it by the end of the day.  The email will look like this:

Your personalized wellness plan and ongoing support

Upon completing the WellU HRA, you will receive a personalized action plan designed to guide you on your health journey.  The action plan will be complete with easy-to-access resources tailored to your specific needs, whether you’re a seasoned athlete or just beginning your wellness path. If your responses qualify you for a tailored care pathway, the Osher Center team will reach out to you to discuss your options. We are committed to making wellness accessible to all university employees, by continuously monitoring access, refining the programs and services offered to fit the needs of employees, and ensuring that we effectively support you with the tools and guidance necessary to help you reach your health goals and maintain your well-being.

Your privacy and security are our top priority

The WellU HRA has been developed using RedCap, a secure survey tool trusted by academic health centers nationwide for handling sensitive survey data. Your personal health data will be kept confidential, with only a small number of the Osher Center for Integrative Health team having access. Access is limited to what is required to provide personalized health advice. Human Resources, university leadership and department leadership will not have access to any of your personal health information—your privacy is a high priority.

Confidentiality and support

Rest assured that all information collected will remain strictly confidential and that your health and safety are our top priority. In the rare instance that someone reports a risk of harm to themselves, the Osher Center team will reach out confidentially, offering assistance and connecting them directly to appropriate care, resources and support. Additionally, in the instance that an employee requests assistance establishing care with a primary care or a mental health care provider, the Osher Center team will connect them with the appropriate care navigators within the U Health system to expedite access to care when possible.
