Form 1095-C now available in CIS

Form 1095-C for 2024 health plan enrollment will be available in CIS on or before March 3, 2025. You do not need this form to file your 2024 taxes. To view your 1095-C form, log into CIS on or after March 3rd and type 1095 in the search bar. Click the “Employee Forms 1095-C” tile and then save or print a copy of the form. If you do not have access to CIS and wish to have a copy of your form, contact HR at AskHR@utah.edu.

Enrollment deadline changes

Beginning April 1, 2025, the deadline to enroll or change employee benefit enrollments will be 30 days (changing from the current 90 days).  If you experience a life change event (including marriage, divorce, death of a family member, birth or adoption of a child) or are a new hire, please enter your enrollment choices in UBenefits as soon as possible, but no later than 30 days following the date of the event to avoid missed deadlines.  As a reminder, a former spouse loses eligibility for coverage on the date the divorce is final, not the date they are removed from the plan.
